CDC Ties Dental Antibiotic Stewardship to Infection Prevention Programs

New CDC guidance positions antibiotic stewardship as part of the infection prevention work already coordinated in many dental practices, asking teams to fold prescribing oversight into their existing infection control programs. The dental-specific document, dated July 1, 2026, adapts the agency’s outpatient Core Elements framework to dentistry.

The CDC recommends embedding stewardship activities in the practice’s infection control manual and aligning them with the responsibilities of the infection prevention coordinator — a role frequently filled by dental hygienists. It calls for a practice-wide system to track antibiotic prescriptions and to require a diagnostic code or documented indication in the dental record, along with engaging electronic dental record vendors to automate that tracking.

Patient and staff education features prominently. The guidance urges teams to educate patients on treatment and prophylaxis recommendations to curb unnecessary use, and to standardize review of a patient’s medical and dental history before an antibiotic is prescribed.

The agency frames the need with prescribing data: general dentists wrote 25.2 million antibiotic prescriptions in 2022, more than 10% of outpatient antibiotic use, and rates have not moved despite updated guidelines. It flags reducing clindamycin, evaluating penicillin allergy labels, using the shortest effective duration, and limiting prophylaxis to a single preprocedure dose when indicated.

Barriers cited include limited access to patients’ medical records, minimal IT support for prescription tracking, and prescriptions that often lack a required indication.

The strategies were issued as CDC document CS364174-B, with prescribing figures drawn from a 2025 analysis in the Journal of the American Dental Association by Huynh and colleagues.
