Blood-Based Cancer Test Could Aid Oral Screening, but Not Replace It

A blood test built on eight serum metabolites detected head and neck squamous cell carcinoma with high accuracy in a 938-person study, a finding relevant to clinicians who screen for suspicious oral lesions and initiate referrals, researchers reported July 13 in EBioMedicine.

The study analyzed serum from 457 patients with head and neck squamous cell carcinoma, 451 controls, and 30 patients with oral potentially malignant disorders. Using liquid chromatography-tandem mass spectrometry and machine-learning feature selection, the team built an eight-metabolite diagnostic model and tested it across discovery, external validation, and independent cohorts.

In internal validation, the model reached an area under the curve of 0.990, with 91.3% sensitivity and 97.1% specificity. In multicenter external validation it achieved an AUC of 0.901, and it held up in Stage I disease at 0.927, suggesting potential for earlier detection.

Performance was weaker at the more clinically difficult task of separating cancer from oral potentially malignant disorders, where the AUC fell to 0.710.

The metabolic signature centered on altered arginine metabolism, marked by decreased arginine and accumulation of ornithine. Integrating transcriptomic and spatial data, the researchers also identified two candidate molecular subtypes with distinct immune features, prognosis, and TP53 mutation status.

Because the analysis drew on existing patient cohorts rather than a real screening population, the authors said the panel requires prospective validation in clinically representative groups before any diagnostic use. The weaker discrimination of premalignant lesions means the test would not replace clinical examination, biopsy, or pathology.

The study was led by researchers at Shanghai Ninth People’s Hospital, Shanghai Jiao Tong University School of Medicine, and collaborating centers in Zhengzhou and Beijing.
