Synopsis:
This course provides a foundational understanding of minimal sedation. It covers essential definitions related to sedation concepts, allowing participants to grasp the distinctive safety aspects of minimal sedation for both dental patients and practitioners. The course also examines the “Spectrum of Sedation,” allowing participants to appreciate the distinctions between minimal sedation and other levels of sedation and anesthesia in terms of patient parameters such as responsiveness, airway, cardiovascular, and respiratory functions.

Dental anxiety and fear remain significant concerns among patients and are prevalent negative psychosocial conditions in the general population. Early studies conducted in the U.S. by Dionne et al. found that 51% of survey respondents reported feeling somewhat nervous, very nervous, or terrified about visiting the dentist. In recent years, sedation and anesthesia in dentistry have gained considerable attention, bringing forth conflicting standards of care and state-specific regulations. Controversies have emerged regarding in-office practice models, safety outcome measurements, necessary training for dentists, as well as the effectiveness and applicability of sedation for patients beyond those with high levels of fear.

According to the latest guidelines from ADA, minimal sedation should prioritize a significant safety margin to prevent unintended loss of consciousness. Typically, minimal sedation is achieved through inhalation and/or enteral methods (such as oral or sublingual medications). The dosing of medication for inducing minimal sedation is usually limited to a single dose or multiple doses that do not exceed the maximum recommended dose for unmonitored home use as set by FDA. Patients under minimal sedation, by definition, should retain the ability to maintain an open airway and respond appropriately to tactile stimulation and verbal commands independently and consistently. While there may be a slight impairment in cognitive function and coordination, respiratory and cardiovascular functions remain unaffected. In most cases, licensed dentists are not required to undergo additional training or obtain certification from state dental boards to administer minimal sedation.

Description of Speaker:
Dr. Jason H. Goodchild earned his undergraduate degree from Dickinson College in Carlisle, Pennsylvania. He then received dental training at the University of Pennsylvania School of Dental Medicine. Currently, he serves as the Vice President of Clinical Affairs at Premier Dental Products Company, where he contributes to the development of innovative new products and provides education to clinicians to enhance their clinical practice. He also holds the position of Associate Clinical Professor in the Department of Oral and Maxillofacial Surgery at Creighton University School of Dentistry and serves as an Adjunct Assistant Professor in the Department of Diagnostic Sciences at the Rutgers School of Dental Medicine. Goodchild maintains a private general dental practice in Elkins Park, PA. He can be reached via email at DrJGoodchild@gmail.com.
